The job itself

The Community Support Team provides services to people living in the community and cover the whole of East Riding.

We are seeking to recruit a keen and enthusiastic individual to join our team of Assessment Officers on a 37hr contract working over a 7-day rota.

The Assessment Officer role is to support customers and their carers out in the community by; completing initial assessments, risk assessing, undertaking medication assessments and reviewing customer’s packages of care to determine any long-term care needs or appropriately sign posting them for alternative services.

The Community Assessment Officer position is essential to the running of the Community Support Services who offer; Discharge to Assess (D2A), re-ablement and are also a provider for the service of last resort for customers in crisis, along with providing essential support for unpaid carers.

The position is crucial to the running of the Community Support service and are integral to meeting the current service demands and service changes.

The successful applicant will:

Have recent experience of working in a social care/community setting

Be able to set realistic goals for customers

Have good IT skills

Be able to travel around the authority
