This role is an integral part of the 'One Recovery Buckinghamshire' (ORB) partnership, a collaborative effort with Inclusion (MPFT NHS Trust) and Build on Belief, ensuring integrated and comprehensive support across the county.

What You'll Do:

  • Engage proactively with rough sleepers and individuals at risk of homelessness, providing holistic support.
  • Address a range of complex needs, including substance use, mental and physical health, relationships, and community integration.
  • Empower clients to secure stable accommodation and access vital health and social care services.
  • Foster collaborative relationships with statutory bodies, voluntary agencies, housing providers, and healthcare professionals.
  • Deliver tailored psychosocial interventions as part of a caring, multi-disciplinary team.
  • Maintain accurate and detailed records of client progress and outcomes.
  • Offer flexible outreach support across community settings.
