The Mental Health Primary Care Network (MHPCN) service is a service focusing on delivering support, advice, assessment and signposting functions to people experiencing mental health issues who present in the General Practitioner (GP) setting.
This is an exciting opportunity to be part of a ‘Whole System Approach’ in the transformation agenda to influence and shape the way that mental health services are delivered in the future along with partner colleagues.
We are looking to employ a Social Worker to a fixed term position (maternity cover), to support in the delivery and development of Mental Health services across the East Riding of Yorkshire - the post would be working across the Beverley and Harthill Primary Care Networks.
As a social worker in this post, you will be part of the East Riding Community Mental Health Team and in-reach to the GP practices. As the Social Worker, you will be part of the Multi-Disciplinary team of a group of PCN professionals ranging from Nurses, Psychologists, Pharmacy, Peer support workers etc delivering the service.
This is a post in a multi-disciplinary team at the forefront of a major service change process and we require applicants who are robust, dynamic and who can operate in a challenging environment whilst continually seeking improvements in service delivery with the commitment to providing a high quality, professional service.
On an operational level this post will work into the GP practice with the main purpose of providing the appropriate Social Work advice and support to people experiencing mental health issues accessing the practice with the focus on providing an effective range of interventions to support a strengths based and recovery based approach for the individual.
There will be an emphasis on ensuring that the social care perspective is central to any assessment, advice or intervention offered and on the statutory obligations and duties for the Local Authority. This will enable the necessary steps to be taken at an earlier stage to prevent, delay or reduce a person’s needs for support and services and thus, promote their resilience and wellbeing in the community. This will include assessing for and arranging social care services where appropriate based on the legislation, reducing inappropriate demands on secondary services and seeking to manage emerging crises for individuals.
Adult Services are progressing a significant change programme, ‘Your Life, Your Way’ which reviews all aspects of our service delivery to promote the strength based approach and supports independence for the people we serve. As part of this, you will be supported on a day to day basis by colleagues in the team and wider service to help further your professional career through training and development opportunities underpinned by the Employee Development Review (EDR) and involvement in the Change Programme.
This role requires travel, and working from different locations. Based at the Beverley and Harthill Primary Care Networks (Beverley and Haltemprice area).
