Job Description: Discharge Coordinator Cromwell Hospital, London SW5 0TU Permanent Up to £32,500 DPE + Excellent Benefits Full time 37.5 hours per week (Shifts run Monday to Friday 8am to 4pm) We make health happen. Role Overview This is a new and vital role in the continuous flow and management of the medical ward, as well as improving patient experience in terms of the discharge process and communication. As the Discharge Coordinator, you will function as part of the multi-disciplinary team within the ward to provide a seamless service & high-quality care to patients under the direct supervision of a Registered Nurse. You will support the ward leadership team and clinical site managers in ensuring prompt, safe and effective daily discharges, as well as documenting clear estimated discharge dates supporting our admission forecasts and enabling us to have continuous flow and capacity for AAU. Youâll help us make health happen by: Be aware of and monitor the progress of those patients who have been declared medically/clinically stable yet continue to occupy an acute hospital bed and expedite the discharge planning progress. Review all patients pre-discharge to ensure they have all the necessary equipment, medications, and information to leave with â improving patient satisfaction and reducing readmission and confusion. Any concerns will be escalated to a registered nurse, and you will support to remedy this within your scope of practice. To access and chase investigations (e.g. blood results / radiology), if these are the primary blocker to discharge. To discuss patients with appropriate departments, enabling assessments to occur which facilitate discharge. Support the ward leadership team in progressing long term patients with their care plan and advocating for the patient at all times. Take responsibility for ensuring all the necessary arrangements are put into place to optimize discharges, including GP letters, outpatient appointment, sick certificate, clothes for discharge and keys available. Ensure medications for discharge are organised and the patient transport arrangements confirmed. To identify any potential equipment needed to expedite discharges and liaise with members of the MDT regarding the appropriateness and ordering of this. To arrange specific tests that could expedite discharges and liaise with consultants/RMO around ordering these. Be the link person and take a key role in assisting self-funding patients to find accommodation in a Nursing or Residential Home and identify links with those organisations who can give impartial advice to self-funders. Key Skills / Qualifications needed for this role: Experience in Coordination within a healthcare environment Experience of working in an inpatient setting Experience within Discharge either private or NHS is desirable Excellent communication skills, written and verbal Able to build strong relationships People person and have a passion for helping people Able to manage own workload with attention to detail IT literate â Microsoft office proficient Benefits Our benefits are designed to make health happen for our people.
